Laser Skin Resurfacing

DOT Laser – What is DOT?

DOT therapy or Dermal Optical Thermolysis is laser-resurfacing technology that is designed to produce refreshed, rejuvenated skin with less down-time and without compromising your outcome. This technology is based on the idea of “fractional resurfacing” that involves the creation of small areas of inflammation, which, when healing, produce a response that generates additional collagen and improves sun damage, fine lines and even acne scarring. It is our treatment of choice for mild to moderate signs of skin aging. Contact our San Diego practice today to schedule a laser skin resurfacing consultation with Dr. Vincent Marin.

How Does DOT Work?

DOT laser resurfacing works on the fractionated approach of laser skin rejuvenation where only a small fraction of the skin is treated at any one time. A treatment pattern or grid is produced on a small segment of your skin, and then the laser produces tiny pulses of energy effectively creating small treatment zones. The settings are adjusted to the particular needs of your skin, the amount of skin, the extent of your fine lines as well as sun or environmental damage. Dr. Marin may make fine adjustments during the course of the procedure to continue to produce the optimal results.
After the DOT laser is fired, the laser precisely creates thousands of microscopic holes or DOTs in your skin. These treatment “DOTs” both create immediate tightening of your skin and produce local inflammation that leads to the stimulation of new collagen growth. This treatment technique is designed to improve texture, tone, fine lines, deep facial rhytids or wrinkles as well as acne scarring. Since the majority of your skin is NOT treated (only a fraction of the skin is treated with the laser), this allows for a more rapid recovery since there are undisturbed intact “islands” of healthy skin around each treatment area.

Who is a Good Candidate?

DOT therapy is safe for nearly any patient who suffers from aging or sun-damaged skin. This technology can help improve fine lines, sun damage, skin laxity and even deeper facial wrinkles and acne scarring.

DOT Procedure

Before undergoing a DOT laser treatment, we will apply a topical anesthetic to the areas we are planning to treat approximately 30 minutes to one hour prior to starting. Oral sedation may be used to help you relax. The laser is then passed over the areas to be treated in a grid-like pattern until all of the skin is effectively “resurfaced.”
After the procedure is complete, your skin will be red similar to a sunburn and have some mild crusting. You will be required to continually moisturize your skin with ointment over the next three to four days. We will provide you with a complimentary Skin Medica Ablative Post Procedure Kit to help treat your skin after the DOT. As your skin begins to heal, you will begin to experience some peeling as the old skin is replaced with a new, refreshed layer of skin beneath. Your skin will look “normal” usually by seven to ten days after the procedure, but you can apply some concealing make-up several days prior (three to four days after the procedure) as you return to work and your normal daily routine.

How Long Does the Procedure Take?

In the vast majority of cases, the entire face and neck can be treated in 20 – 30 minutes depending upon your comfort level. The timing will be either longer or shorter depending upon how much area is to be resurfaced.

How Many Treatments Will I Have to Undergo?

Most of our laser skin resurfacing patients only need one treatment with the DOT laser. However, if your fine lines or environmental damage is significant, it is possible that you will need one to two more sessions with the DOT laser. We will likely be able to predict this during your initial consultation.

How Do I Maintain My Results?

The most important aspect of good skin care is the regular use of SPF 30 sunscreen, and this should be regularly applied on a daily basis. We recommend a combination of a hydrating moisturizer, topical anti-oxidants, growth factor and peptide cosmeceuticals with a retinol and limiting your sun exposure. This combination of skin care should leave you with results that last years and minimize the preventable causes of premature aging.

Is DOT Safe?

DOT laser therapy is approved by the FDA for facial resurfacing. Since IV/IM sedation is not required, there is no risk associated with this practice or an unforeseen reaction to the medications used. DOT lasers are extremely flexible and allow for precise control and modifications depending upon which areas are being treated. As only a fraction of the skin is being treated at only one time, this allows for more rapid healing process. As the DOT laser does not penetrate as deep as many “fractional” lasers the consequences are less, but it still is able to deliver maximal results with a rapid recovery.
