Browlift Candidates
The eyes and brows are viewed together as a single unit, as both of these anatomic areas contribute to an attractive, youthful upper eye or make the eye look “tired” or give an “angry” appearance. This can be congenital, meaning that the patient was born with this shape or acquired with age. Good browlift surgery candidates are patients who have a lower brow position, a “heavy” upper eyelid or those who have a shape that gives the appearance that the patient is unhappy or angry.
Browlift Goals
An attractive and youthful brow is defined by not just the height of the brow, but also its shape and position. Youthful and attractive models can have a low position of the medial brow (toward the nose) but still look good if the brow gradually arches as it progresses to the side of the face – just look at any cosmetic advertisement in a fashion magazine. Our goal is to open the eye and improve the shape and position of the brow itself. This combination will lead to an open, attractive and youthful upper lid. Merely elevating the brow without concern for shape can result in an “over-elevation” and a higher brow, but not necessarily one that is attractive. I can demonstrate this on the patient during a consultation to ensure that we are both in agreement as to our ideal goal.
An additional benefit of a browlift surgery is that the frown muscles can be weakened at the time of surgery. This can soften the unwanted “11” frown lines in the area between the brows and diminish the amount of BOTOX injections needed to soften this area.
Browlift Techniques
There are several surgical techniques commonly employed to elevate and reshape the brow: endoscopic, coronal and hairline approaches.
The endoscopic technique employs several, small incisions hidden in the hairline to access the appropriate surgical areas. A small camera is used to visualize the anatomy and release the brow ligaments and visualize the removal of muscles if needed. This technique is particularly suited to a younger brow lift patient who needs a smaller lift and primarily reshaping of the brow.
A “coronal” approach uses an incision across the top of the head (from ear to ear) and is the strongest and most effect approach to elevate the brow. This technique provides for complete visualization and strong fixation to ensure a long lasting result. The incision is longer, but is very well hidden in the hair so that not even a hairdresser would notice. One limitation with a coronal approach is that the hairline can be elevated in addition to the brows. When this is an issue, a hairline approach is often the best choice.
Hairline brow lift surgery is a wonderful alternative to a coronal approach where the hairline needs to be either maintained or even lowered in someone with a particularly high forehead. An incision is made along the hairline and the brow can be elevated, the frown muscles addressed without moving the hairline whatsoever.
Each of the techniques carries similar risks and the appropriate choice is based upon your particular anatomy, needs as well as ultimate aesthetic goals.
Browlift Recovery and Results
Most browlift techniques are outpatient surgeries performed under general anesthesia with a Board Certified anesthesiologist. Our San Diego browlift patients will have some swelling, mild bruising and notice some degree of numbness in the brow region during the recovery period. This is almost always temporary. Pain is limited and more traditionally described as a headache rather than typical surgical discomfort. Most incisions are either well concealed in the hairline or easily hidden allowing for a relatively rapid return to work. You will notice your results immediately, but the final appearance is usually within six to eight weeks and scars predictably mature over time.
